Субклинический гипотиреоз у беременных и детей: диагностика и лечение с учетом современных рекомендаций

Аннотация

Субклинический гипотиреоз — наиболее распространенный вариант тиреоидной дисфункции. У большинства взрослых он протекает бессимптомно и нормализируется без лечения. Поэтому гайдлайны авторитетных эндокринных обществ рекомендуют тактику наблюдения и выборочного лечения пациентов с дополнительными факторами риска. Но для беременной любая, даже минимальная, тиреоидная дисфункция создает риск неблагоприятного течения беременности и родов, а для ребенка — задержку нервно-психического развития. Поэтому Европейская тиреоидная ассоциация рекомендует более жесткие референсные интервалы для ТТГ и активное лечение субклинического гипотиреоза во время беременности и до трех лет жизни ребенка, а в первом триместре беременности — даже изолированной гипотироксинемии.
