Концепция замещения рецепторов как предиктор клинической эффективности препарата: сравнительный анализ блокаторов Н1-рецепторов второго поколения

Аннотация

Антигистаминные препараты второго поколения являются терапией первой линии в лечении хронического ринита и хронической идиопатической крапивницы. Большое значение имеет оценка фармакокинетических и фармакодинамических свойств препаратов и их клинической эффективности, однако точные модели для прогнозирования последней на данный момент отсутствуют. Показатель степени замещения рецепторов является предиктором фармакодинамики и антигистаминного потенциала препарата, отображает его сродство к рецепторам и плазменную концентрацию. Таким образом, замещение рецепторов может рассматриваться как более точный показатель клинической эффективности препарата. Данное исследование было проведено с целью изучения корреляции показателя замещения рецепторов и клинической эффективности препарата на примере антигистаминных препаратов второго поколения. Обзор литературы проводился с использованием MEDLINE. Поиск осуществлялся по терминам: аллергия, аллергический ринит, эффективность лекарственного средства, запатентованные лекарственные средства, многолетний аллергический ринит, сезонный аллергический ринит, второе поколение антигистаминных препаратов, хроническая идиопатическая крапивница, результаты лечения. Также были использованы тезисы и информационные материалы последних съездов и конференций на тему аллергии. Показатель замещения рецепторов для антигистаминных препаратов второго поколения был получен в сравнительных исследованиях. Показатель замещения рецепторов у левоцетиризина был выше в первые 4 часа после приема по сравнению с дезлоратадином. Данный показатель оставался выше у левоцетиризина через 12 и 24 часов по сравнению с дезлоратадином. Показатель замещения рецепторов этих препаратов коррелировал с показателями фармакодинамики, такими как кожные и общие проявления аллергии. На показатель замещения рецепторов влияет время от момента последнего приема препарата, рН и режим дозирования. Между замещением рецепторов и концентрацией препарата в плазме крови определяется нелинейная зависимость. Результаты исследования указывают, что RO является точным предиктором фармакодинамической активности и клинической эффективности in vivo.
